We are always trying to improve our quality of care for patients. Please assist us by completing the following survey and tell us about your experience of using our Maternity services.

All responses will be treated anonymously unless you specifically request to be contacted.
1. During your pregnancy were you given enough direction to useful sources of information contained within your hand held record, such as the NHS Choices website?
2. Were you given a choice about where your ante-natal check-ups would take place?
3. Thinking about your antenatal care, were you involved enough in decisions about your care?
4. If you attended ante-natal classes, was the location convenient for you?
5. If you attended antenatal classes was the timing of the sessions convenient for you?
