Hello I am a Stage One Health Psychology student at the University of Central Lancashire and would be grateful if you would consider participating in my research project, by completing the following questionnaire.

If you agree to take part in this study, you will be asked to answer questions concerning your health and aspects of your lifestyle that may have important influences on the way in which people behave, and their health and wellbeing. The questionnaire should take approximately 15 minutes to complete, some of the questions will ask sensitive information about your own health and if you have any personal experience of cardiovascular disease. Please note that participation in this study is completely voluntary, if you feel that you may be upset by this area of research, please do not participate. If you do not feel that you can answer a question please leave blank, although we would like you to attempt each question. If you feel distressed by any area of this study, there are sources of support at the end of the questionnaire. Background information is taken in order to reflect the balance of different groups within the study.
Once more please be aware that participation is completely voluntary and that you are free to withdraw at any point until you press the submit button. As this study is anonymous, it will not be possible for you to withdraw once you have completed your questionnaire and pressed submit, this is because it will not be possible to identify individual responses. Please read the instructions on the top of each page as the individual requirements may differ. The answers that you have provided are anonymous as your name has not been requested. Please be aware that once submitted, the questionnaires will be stored in a secure and locked location. Anonymous data will be used for research purposes and averaged results may be reported at future conferences or as publications in academic journals.
