We are researchers at University of Maryland College Park conducting a study on factors related to requests of job accommodations. We are interested in your perception on factors affecting job accommodation requests.
You are invited because of your role as an employee with a disability who needs accommodation(s) in your workplace. Your responses to this study will help service providers and employers learn more about factors affecting job accommodation requests, and to help improve future services for individuals with disabilities.
Filling out this 20~30-minute survey is voluntary. You may decide to participate or quit at any time. You will not be penalized or lose any benefits to which you otherwise qualify.
To protect your rights in this study, the researchers will adhere to confidentiality standards at the University of Maryland.
After completing the survey, you may opt in to participate in a raffle for a chance to win a $25 gift card (first 100 participants) or a $10 gift card (rest of the participants). One of every four participants who complete the survey will receive a gift card.
