This consent form is an agreement on your part to participate in research about the parenting style of fathers. This study involves research which will be conducted by Nicole Preiss, M.A., a Psy.D. clinical psychology student at the California School of Professional Psychology, Fresno (CSPP-Fresno) at Alliant International University. You will be asked to answer questions about possible sexual abuse and physical abuse you may have experienced in childhood. You will also be asked to answer questions regarding your own parenting practices and behaviors. There will be questions that will ask you to identify if your own parents struggled with alcohol abuse. Finally, you will be asked questions that relate to your current social supports.

You are asked to participate because you are 18 years of age or older, residing in the United States, are a father regardless of personal abuse history, and have at least one child (biological or not biological) who is three years old or older.  If your child(ren) is not your biological child(ren), then you at least know the child(ren) for three years. Your participation in this study will involve the completion of a series of questionnaires that will take approximately 30-60 minutes.

The risks to you in this study are that some of the questions may make you feel uncomfortable. If you suffer any ill effects as a result of your participation, please inform the researcher or the research office. If while completing the questionnaires, or after, you feel the need to talk with someone regarding thoughts or feelings related to these questions, please let the researcher know and you will be assisted.  If necessary, you will be referred to a therapist.  However, neither the researcher nor CSPP-Fresno will incur the cost.  In addition, a list of referral numbers will be provided at the end of the survey if you wish to seek psychological help at your own expense. These referral numbers can also be provided to you by the researchers at your request, at any time.

Your participation in this study is completely voluntary and you may withdraw at any time without penalty. After you complete the survey, your data will be automatically tabulated with all others and it cannot be identified or withdrawn. Your identity as a participant in this study will be kept in strict confidence and no information that identifies you in any way will be released.

By selecting 'accept' below you are giving me permission to collect psychological data from you as a participant in this research project.  Please be aware that this information will be kept confidential and that I will follow the American Psychological Association Ethical Standards including those for Research with Human Participants.

Only Nicole Preiss, M.A. and Jennifer Ivie, Ph.D. will have access to your responses, which will be combined with other participant data so individual responses will not be able to be identified. The researchers will hold all data collected for five years, after which time it will be destroyed.

Although you may not directly benefit from this study, your participation in this project will produce information that may be of benefit to boys who experienced childhood sexual abuse and their future parenting practices. Other benefits are that you may find the material interesting and receive some benefit from knowing that you contributed by participating. This is an opportunity to speak out in a non-threatening, safe situation. Your participation helps break the culture of silence around child abuse.

If you have any questions about this project or your participation in this study, you may contact Jennifer Ivie, Ph.D. at jivie@csufresno.edu or Nicole Preiss, M.A. at npreiss@alliant.edu. You may also call the Research Office at the California School of Professional Psychology, (559) 456-2777, Ext. 2309. You may also request a summary of results or additional information about the study from Nicole Preiss, M.A. at the same contact information. Thank you for your participation!
