Statement of Informed Consent

The University of Bolton supports the practice of protection for human subjects participating in research. All research is supervised and has passed the University Ethics Committee.

I am a student at the University of Bolton completing my 3rd year project under the supervision of Abigail Thornton. This study is designed to collect information on levels of academic anxiety and to determine locus of control orientation in university students. This will be done by asking participants to complete Rotter's locus of control scale and an academic anxiety scale inventory You will also be asked to state if you have been diagnosed as having dyslexia or not.
If you choose to fill out these instruments, you will be answering questions about your general beliefs and feelings towards your academic studies.

All information received will remain strictly confidential and no one will have access to it outside of the individuals involved in this research. The estimated time required to complete this questionnaire is approximately 5-10 minutes.

Your participation is solicited on a strictly voluntary basis. Participation is strictly confidential. Your name will not be associated with the data, as the findings will be studied as a group. You can withdraw from this study at any time until the point you click submit, and all of the information will be removed from the study and destroyed. Once you have submitted your data it cannot be withdrawn due to the anonymity of the data as it will not be possible to identify individual responses.
